We are making publicly available the questionnaire used for the "Global Behaviors and Perceptions in the COVID-19 Pandemic" project, which collects data from a large-scale online survey about COVID-related behaviors, beliefs, perceptions, mental health, and more, covering respondents from more than 170 countries. The data was collected via snowball sampling starting March 20, 2020 through a survey instrument that was translated by volunteers into 69 languages. The project offers a unique snapshot on the evolution of behaviors and perceptions about the pandemic on a global scale in the early and accelerating phases of the COVID-19 outbreak. More data collection is ongoing. This short (10-15 minutes) survey is meant for organizations on the frontline of education service delivery (school operators, other education service providers, organizations delivering children's rights, childcare and/or gender equality programs in education systems, etc.). The purpose of the survey is to understand what these organization's biggest concerns and challenges for girls are in light of COVID-19 school closures. This questionnaire is designed to be administered to smallholder farmers across various project locations. The survey collects information across various areas, including COVID-19 related market disruptions, input availability, food security, knowledge about COVID-19 and perception of public health advisory. Questionnaire partly adapted from the IGC Economic Impact Questionnaire (https://docs.google.com/spreadsheets/d/14Dl54dpD37pgEDdir9uyHqHNHwtrMg96ZQTZQGcWC5Q/edit?usp=sharing) This is a core survey instrument that is intended to be adapted by projects to suit local contexts. The questionnaire was designed to survey smallholder farmers in developing countries. Questions may need to be adapted if administering to a different group.
